Evaluation forms
As an employer of a co-op student, you will complete and submit a formal performance evaluation on your co-op student by the end of their work term so that they will receive academic credit. The performance evaluation remains confidential to you, your student, and Co-operative Education, but the overall rating you give your student will become part of their co-op work history, which is viewed in future terms by potential employers during the job application or interview process. Completing an informal midterm evaluation between you and your student is also encouraged.

Work report evaluation forms
Students are responsible for writing a work term report after they complete a work term. If you hired a Mathematics or Applied Health Sciences student, they will likely ask you to read the final version of their work term report and evaluate it using the forms below.
