Researchers from the University of Waterloo, Western University and Conestoga College will host a community discussion on Tuesday about the risks associated with transitioning older adults between health-care settings.

Organized as part of the Canadian Institutes of Health Research’s Café Scientifique series, the event will discuss the challenges patients face when moving from one setting to another, such as from hospital to home care. With current practices, many patients report feeling excluded from the decision-making process, rushed and ill-prepared to adapt to their new environment.

Join leading experts to explore what patients, family members and health-care providers can do to make transitions for older adults smoother and to improve care.

Panelists include Professor Paul Stolee, School of Public Health and Health Systems at the University of Waterloo; Dr. George Heckman, lead geriatrician for the Waterloo-Wellington Local Health Integration Network and professor at Waterloo; Professor Bert Chesworth, School of Physical Therapy, Western University; Professor Veronique Boscart, Schlegel industrial research chair in seniors care for Colleges; and Margaret Hedley, consumer perspective, Guelph-Wellington Seniors Association.
