Managing the Seams: Transitions in Health Care for Older Adults

Older adults with complex health problems, such as those with hip fractures, often need care from multiple health professionals across a variety of settings. More and more it is being recognized that care quality and patient safety may be most at risk during the transition from one setting to another.

Despite the frequency and importance of these transitions, transitional care has received little attention in research, health policy, and clinical practice. The InfoRehab Transitions project focused on the transitional care experiences of older hip fracture patients, and involved interviews with patients, their family members, and health care providers at each transition in care.

Join us to discuss what the project discovered about what is currently working well, what areas need improvement, and what patients, family members, and health care providers can do to make transitions as smooth as possible.
