Thank you for taking part in the Waterloo Region Community Wellbeing Survey. By completing this survey, you will be helping Waterloo Region improve the quality of life for all residents.
The results from the survey will help develop polices and improve services and supports for people living in Waterloo Region. The Canadian Index of Wellbeing (CIW) is administering the survey on behalf of 16 partner agencies in Waterloo Region.
About the Canadian Index of Wellbeing (CIW)
The CIW is based in the Faculty of Applied Health Sciences at the University of Waterloo and, through its research and policy recommendations, it aims to promote wellbeing and improve the quality of life of all Canadians.
About the survey
The survey has been designed to gather information concerning the eight domains that the CIW has identified as critical to our overall wellbeing: community vitality, healthy populations, democratic engagement, the environment, leisure and culture, education, living standards, and time use. The questionnaire takes about 30 minutes to complete, and provides a comprehensive look at all aspects of wellbeing for residents living in Waterloo Region.
Your participation in this study is entirely voluntary and involves only the completion of this online questionnaire. All of the information that you provide in the survey will be kept strictly confidential. You are not asked for your name or address at any point during the survey. The data gathered will be presented in a report that will summarize the findings of the survey and will be made available to the public on the Wellbeing Waterloo Region website.
All the findings will be reported at a level of geography that will not identify individuals (e.g., for the entire region or for individual townships). Additional protections and rights are described in the “Consent and Information Release”.
This project has been reviewed and received ethics clearance through a University of Waterloo Research Ethics Committee (ORE #23012). To complete the survey, please have one person in your household who is 16 years of age or older and whose birthday is closest to June 1st follow the "Start Survey" link below before June 11, 2018.
