Welcome to the Wood Buffalo Community Wellbeing Survey! You will need the unique 5-digit code from your invitation letter in order to start the survey.

Thank you for taking part in the Wood Buffalo Community Wellbeing Survey. By completing this survey, you will be helping to improve the quality of life for all residents.

The results from the survey will help develop polices and improve services and supports for people living in the Regional Municipality of Wood Buffalo. The Canadian Index of Wellbeing (CIW) is administering the survey on behalf of the local community organizations listed above. 

By beginning the survey, you acknowledge that you have read this information as well as the “Consent and Information Release” and understand and accept the proposed uses of the survey information you will be providing.

Your opinions are very important to us and so is your time. If you are unable to complete the entire survey in one sitting, take a break and come back later to finish. We’re happy to wait!

About the Canadian Index of Wellbeing (CIW)

The CIW is based in the Faculty of Applied Health Sciences at the University of Waterloo and, through its research and policy recommendations, it aims to promote wellbeing and improve the quality of life of all Canadians.

About the survey

CIW graphic of eight domainsThe survey has been designed to gather information concerning the eight domains that the CIW has identified as critical to our overall wellbeing: community vitality, healthy populations, democratic engagement, the environment, leisure and culture, education, living standards, and time use. The questionnaire takes about 30 minutes to complete, and provides a comprehensive look at all aspects of wellbeing for residents living in the Regional Municipality of Wood Buffalo.

Your participation in this study is entirely voluntary and involves only the completion of this online questionnaire. All of the information that you provide in the survey will be kept strictly confidential. You are not asked for your name or address at any point during the survey. The data gathered will be presented in a report that will summarize the findings of the survey and will be made available to the public on the FuseSocial Wood Buffalo Society website.

All the findings will be reported at a level of geography that will not identify individuals (e.g., for any of our communities including Fort McMurray or for RMWB as a whole). Additional protections and rights are described in the “Consent and Information Release”.

This project has been reviewed and received ethics clearance through a University of Waterloo Research Ethics Committee (ORE#41004). In addition, all name and address information is kept strictly confidential. Canada Post prepared the mailing list for the survey invitation letters and then delivered this directly to a printer who signed an agreement to keep these details confidential. None of the local community organizations listed above, or the Canadian Index of Wellbeing has access to the names and addresses.
