My Active Health supports UW WELL-FIT@HOME
The 12 week UW WELL-FIT@HOME program focuses on restoring and improving the physiological well-being of women with breast cancer currently undergoing chemotherapy, radiation, or hormonal therapy treatment. This program has been funded by the Quebec Breast Cancer Foundation and Ultramar Grant in support of My Active Health Ontario.
The effects of cancer treatment are wide ranging and individualized. Trained exercise specialists consider medical history and health status when developing individualized exercise programs aimed at improving cardiovascular function, balance, muscular strength, and endurance as well as range of motion at specific joints.
The specialized REMOTE EXERCISE program includes a pre and post fitness assessment, 2 personal training sessions to review individually designed exercise programs, behaviour modification modules, goal setting and 8 weekly check-ins with CCCARE Fitness Staff. Appropriate for most individuals regardless of fitness level living in the Waterloo-Wellington region, UW WELL-FIT@HOME tailors each program to the individual's specific health and fitness, as well as personal goals. All individuals must be able to safely exercise independently at home without Fitness Staff supervision.

WHO IS A CANDIDATE FOR THE UW WELL-FIT@HOME PROGRAM?
Candidates for the UW WELL-FIT@HOME exercise program include women currently undergoing any form of treatment for breast cancer* or are within 2 years of completing chemotherapy, radiation, immunotherapy, surgery or starting hormonal therapy treatment and living in the Waterloo-Wellington region.
For others not included in the determined criteria, but still feel the program may benefit them, please contact CCCARE Fitness staff who will assess status on a case by case basis.

REFERRAL TO PARTICIPATE
Prior to participating in the UW WELL-FIT@HOME program, participants require clearance from their oncologist. Interested participants should ask their oncologist to complete and submit the "2021 Referral Script: UW WELL-FIT Remote Exercise Program" if unsupervised exercise is deemed appropriate.
These scripts/referral forms are available at Grand River Regional Cancer Centre (GRRCC), and can also be obtained by speaking with supportive care staff. Once this referral script is complete, CCCARE Fitness Staff will be in touch to book an assessment.
For those not being treated at GRRCC or are no longer under the care of an oncologist, please contact CCCARE Fitness staff for a copy of the referral script to be filled out by treating oncologist or family doctor.
WHAT HAPPENS NEXT?
The program begins with an initial remote consultation and exercise assessment with each participant. This initial appointment takes up to 1.5 hours to complete.
During this remote appointment, CCCARE Fitness staff obtain a detailed medical and physical activity history, and functional movements such as range of motion, balance, and strength as well as collecting self-reported measures height, weight, resting heart rate and blood pressure. To obtain a measure of current physical activity, each client will complete the DUKE Activity Status Index and a survey related to physical activity.
In preparation for the remote program, CCCARE Fitness staff will provide contactless pick up of a Fitness Kit (yoga mat, resistance bands, pedometer, and Workbook).
The 12 week UW WELL-FIT@HOME program consists of 2 personal training sessions to demonstrate individually designed exercises, and 8 weekly check-ins to work through behaviour modification modules and set goals. Each client will complete the exercise program independently at home. CCCARE Fitness staff will be available to assist and answer any questions during the weekly check-ins.

Information for UW WELL-FIT@HOME participants
All information regarding directions and preliminary instructions to follow on the day of the assessment are outlined below and any additional information will be emailed directly. For additional questions, please contact CCCARE Fitness staff for clarification.

What to have:
- Please have a list of all prescribed medications, including dosage information, that you are currently taking.
- Short sleeved/sleeveless shirt and shorts/track pants/yoga pants should be worn (something you can move comfortably in). Running shoes are the recommended footwear.
- CCCARE Fitness Staff will arrange a contactless pick-up of the kit at the address listed below.
