Accommodations for Midterm & Final Exams

Deadline to submit white copy of Alternate Examination Arrangements for midterm/test accommodations:  7 days before the first midterm

If a midterm is listed to take place in less than 7 days from the date of submission, it will not be accommodated. 

Deadline to submit white copy of Alternate Examination Arrangements for final examinations: July 10 2015

If a white form is submitted after this date, there is no guarantee that final exams for Spring 2015 will be accommodated and the completion of a Request for Late Alternate Exam Arrangements form is mandatory.
