Camp Pick-up Form

It is Waterloo Engineering Outreach’s policy is that only the parent and/or legal guardian on file can pick up a child from our programs. We understand however, that, at times, the parent on file cannot be the one to pick up the child for a variety of reasons. If you wish to have another adult be able to pick up your child please fill out the following.

Please note that your child will not be permitted to leave the program until they are either: (a) picked up and signed out by the parent and/or legal guardian on file or an individual authorized by the child’s parent and/or legal guardian as it pertains to the participation of their child in the Engineering Outreach Programs; or (b) if your child is  twelve years of age or older and you have completed the permission to leave unescorted section at the bottom of this form.

[If there are specific custody/access restrictions, please advise the Engineering Outreach Coordinator in writing, with pertinent legal documentation.]
