Pre-Camp: For the December 6th PD Day, please fill out this form by 4pm on December 5th to ensure the name is added to the list. This is so we have some time to process all submissions and add them to our registrations in preparation for the camp session.
Any forms filled out after this time will not be on our list at drop off.
Sometimes the people who register their child for camp are not the child’s legal parent or guardian (for example, a grandparent, aunt, or other relative registers the child since they will be visiting them that week). We know when this was done, it was done with the knowledge of the actual parent/guardian. You have indicated that while you are not the Camper’s parent or legal guardian but have obtained their parents’ or legal guardian’s authorization as it pertains to the Camper’s participation in the Engineering Outreach Program (the “Program”). Safety of the Camper is our number one priority. In cases of emergency or other communications regarding the Camper, it is our policy to contact the Camper’s parent and/or legal guardian to ensure that they are apprised of the Camper’s participation and well-being at all time during the Camper’s participation in the Program.
Please note that after this form is completed, we will be sending a confirmation email to both you and the Camper’s parent(s)/legal guardian you’ve indicated below to confirm: (i) the Camper’s registration; (ii) the names of the authorized individual on the Camper’s file; and (iii) to confirm your authorization to act on their behalf regarding the Camper’s participation in the Program and other matters which may arise during this time (i.e. . filling out additional pick up information forms, self-sign out waivers, and/or contacted in case of an emergency).
