## 1. Survey Participation and Use of Information

1.1 Survey Participation

Participation in the survey is confidential and voluntary. You may decline to respond to any question by leaving it blank or withdraw from the survey at any point by not submitting your responses, with no impact on your ability to participate in the incentives draw. (See incentive rules and regulations below for more information).

1.2 Use of Survey Results

The Graduate Student Association – University of Waterloo will use the results of the survey to develop a five-year strategic plan, which will help inform service and advocacy efforts so that we can work to better support graduate students at the University of Waterloo. Any public report that is made in support of this will include only summarized results to ensure that no individual student can be identified.

1.3 Use of Personal Information

This survey collects identifying information. This information will be used for authentification purposes. Contact information used for the purposes of the draw will be stored separately from survey responses and deleted once the draw is complete and awards have been collected. If you are not participating in the survey, by entering the contest and voluntarily providing personal information including name and email, you are granting us permission to collect and use the information for the purpose of administering the contest and selecting winners.

1.4 Data Storage

The data collected from the survey will be kept on a secure and password protected hard drive. Only the principle investigators will have access to this information. Data will be kept for 7 years, after which point it will be destroyed.

Any identifying information that is collected in this survey will be used for authenticating responses and will be destroyed thereafter.

1.5 Data Sharing

Data collected from the survey will not be shared with any third party organization in the raw form. Any information that is shared will include only summarized results.

## 2. Incentive Rules and Regulations

2.1 Eligibility

Students who submit their responses to the Grad Life: Vital Signs survey will be prompted to enter into the random draw for 1 of 18 $100-dollar gift cards (3 awarded per faculty) after submitting their responses. Eligibility for the incentive opens on Monday January 9th, 2017 at 9:00am and closes on Friday, February 3rd, 2017 at 5:00pm. Please note that you do not need to participate in the survey to be eligible to participate in the draw. To be included in the draw, please send your name, email and University of Waterloo Student ID to the GSA Office (gsaoffice@uwaterloo.ca) by Friday, February 3rd, 2017 at 5:00pm. 2.2 Right to Disqualify The Graduate Student Association – University of Waterloo reserves the right to disqualify any entry not conforming to these Rules and Regulations at any time. The Graduate Student Association – University of Waterloo assumes no responsibility for entry fraud committed by an entrant and reserves the right to demand return of the prize and all costs associated with remedying any prize awarded to an ineligible entry or entrant. The Graduate Student Association – University of Waterloo is not responsible for any incomplete, failed or delayed transmission of your submission due to the internet, including interruption or delays caused by equipment or software malfunction or other technical difficulties. 2.3 Winner Selection The winners of the 18$100-dollar gift cards will be selected in a random draw from eligible entries on Wednesday February 15th, 2017. Your odds of winning one of the 18 $100-dollar gift cards will depend on the number of individuals that enter into the draw. Students will be eligible to win one of three gift cards from their respective faculty. The award winner will be contacted by the Graduate Student Association – University of Waterloo within ten business days of the draw. Please note that the selected entrants for the 18$100-dollar gift cards must correctly answer a time-limited skill-testing mathematical question.

3. Other

The award amount is taxable. It is your responsibility to report the amount received for income tax purposes.
