Graduate House Group Booking Request Form

Thank you for considering the Graduate House for your group event. 
The Grad House can host your group event for 10 - to - 50 people. Group bookings require one week notice

If you will be providing food for this event, please review the Group Booking Menu and then note the items in the space provided "menu options" on the form below. 

The Grad House accepts cash, cheque, debit, visa and mastercard for payment.  If you are authorized to pay with the University's Unit4 Account, then please download the form and indicate that you will be paying by Unit4. Please sign the form in the designated space and send it along with the organizer to facilitate internal billing. (Note: The GSA is not included in the internal Unit4 billing structure.)

Please note that events not solely for graduate students may incur a rental charge and completing this form does not guarantee your group booking.

The Grad House accepts Cash, Cheque, Visa, MasterCard, Debit. If you wish to pay by Unit4 PRINT A COPY OF THIS FORM, sign and make sure the organizer brings it to the event.
Type your name, print the copy emailed to you, sign and send it with the event organizer. Signatory must have billing authority for this account.
