DESCRIPTION:In this talk I will present an overview of my research on chron
 ic care\nservices.  I will then focus on the problem of care delivery for
 \ncomplex patients\, with multiple comorbidities. In this project\, we\nde
 velop a Markov Decision Process framework to manage care for\nindividual p
 atients with multiple chronic conditions through a complex\ncare hub. Comp
 lex care provision influences the evolution of Patient\nActivation Measure
  (PAM)\, an indicator for healthy behavior\, which\naffects the evolution 
 of health state of patients.
