The Registrar's Office requires a cover page for all final examination masters printed through the Registrar's Office. The cover page is the first page of the exam master. The cover page is printed on white paper. The instructor must specify on the cover page whether the exam is closed book, open book, or special materials. If designated as special materials then the instructor must specify on the cover page what materials are allowed. All exam papers will be printed double-sided unless the second side of the sheet is designated for rough calculations as indicated on the cover page.
There are three different ways to create the exam cover page: word document, filling out an Adobe PDF form, or creating your own.

Save the Exam cover page form (docx). If you need to change the marking scheme section it can be deleted or modified.
Filling out and printing the exam cover page (pdf)
You can fill out the Exam cover page fill-in form (pdf) online and print it, or print the form and fill it in manually. A reset button has been added. The reset button will not display when the page is printed.
Creating your own exam cover page
You may create your own exam cover page as long as all of the information as indicated in the Exam cover page sample form (pdf) is included in your cover page.
