Canadian Low Vision Achievement Award

The inaugural Canadian Low Vision Achievement Award will be presented at the Conference in March 2017.

Criteria

This biennial award is presented to an individual who has made a significant contribution to the promotion, development, improved delivery, or knowledge of low vision and/or blindness rehabilitation within Canada. Contributions can included research, clinical, service, and / or education-related achievements. The award will be presented at the Canadian Interdisciplinary Vision Rehabilitation Conference.

Nomination Instructions

Nominations should include name, institution, and contact information for the nominee and a description in 500 words or less outlining why the nominee is worthy of the award. While not mandatory, a CV/resume will assist the judges in making a final decision. Please use accessible formats such as word processing or plain text documents.

Selection Committee

Nominations will be reviewed and judged by a committee made up of experienced representatives from each profession in the field of low vision rehabilitation (cnib, ophthalmology, optometry, occupational therapy, opticianry, teaching) and an individual with low vision.

2017 Deadline

The deadline for receiving nominations is January 31, 2017.
