Intro

Hello, my name is ____________. I am calling from the University of Waterloo Survey Research Centre. We are conducting a research survey on ___________on behalf of researchers at _______________ and we'd like to conduct an interview with someone in your household.

The survey should take about _____________ minutes to complete.  We are not asking for money or selling anything. In order to randomly select individuals, we’d like to speak with the adult in your household, age 18 or older, who has the next birthday. Would that person be available?

  1. Yes, once eligible person on phone, reread introduction then go to Ethics and Consent
  2. Child – no adult available go to child
  3. No adult in household go to ineligible
  4. No/respondent refuses, end interview
  5. Respondent questions criteria or wants to participate without meeting next birthday criteria: We need to select someone at random. With each call we make, we ask to speak to the person whose birthday is coming up next. This helps us to ensure that we have a representative sample as some groups of people are less likely to answer the phone.
