Student WatCard request form

This webform is for new students to request their WatCard. They will be able to select either digital or physical WatCard depending on which better fits their circumstances for the term. If they are living in the Waterloo area and are looking to use their WatCard to access student services such as; UPass, athletic facilities, Food Services, or access to residences then they will need to recieve a physical card. This form should be filled out no earlier than six weeks prior to your first academic term.
