Employees may access and correct their own personal records by contacting occupationalhealth@uwaterloo.ca (subject to legal exceptions). We will respond to your request for access to your record as soon as possible. If we are not able to respond within 30 days, we will contact you and let you know in writing the reason for the delay.
To request a copy of your medical documentation on file with Employee Health and Accommodations (EHA) complete the Authorization to Release Documentation Form and email the completed form to occupationalhealth@uwaterloo.ca.
