Library card activation

Faculty, staff and postdoctoral fellows employed at the University of Waterloo or the federated and affiliated colleges and universities may use this form to register with the Library or update information.

If you are a visiting faculty member, visiting scholar, or a staff member working directly for a faculty member, you will need to include your host's contact information in the departmental point of contact field.

Complete this form if you:

  • Have a UWaterloo WatCard but have not yet registered with the library.
  • Wish to have some information on your library patron record changed.
  • Need to provide some additional contact information in order to enable single sign on to library resources.
