Eligibility and Enrolment Form
To ensure that the views expressed during the citizen assembly on June 6th are from a group of Canadians who reflect the country’s demographic diversity, you will need to complete this form to enrol. Please note that it is possible that the results from the citizen assemblies may be used in
research reports, but your contributions will be kept anonymous. If you enrol, we will take this as a sign that you understand that your contributions during the June 6th event may be used for research purposes. The enrolment form should take about 5 minutes to complete and the questions are quite general (e.g., What is your age group?).
The information provided in this form is confidential and will only be used to select a group of 100 participants to attend the citizen assembly. A citizen information letter on this event can be found here.
JUNE 2nd UPDATE: WE ARE STILL ACCEPTING ONGOING APPLICATIONS from Canadian citizens meeting any of the following criteria:
- Aged 18-49
- High school education or less
- Living in a rural area
- Not in the work force for any reason OR working in the trades, transportation or equipment operators sectors OR working in the service industry
- Aboriginal OR Southeast Asian (including Chinese, Japanese, Korean, Filipino) ethnic background.
