Canadian Frailty Network

Canadian Frailty Network (CFN), funded by the Government of Canada's Networks of Centres of Excellence (NCE) program, is Canada's network focused on frail elderly. Their mission is to improve the care of seriously ill, elderly patients and support their families and caregivers.

Strategic priorities

CFN's strategic plan centres around four strategic priorities, developed through consultation with key partners and stakeholders:

  1. Foster matching care to values
  2. Empower, engage and support patients and their families/caregivers
  3. Improve clinical outcomes
  4. Improve care across the continuum

CFN provides partnership between 40 Canadian universities, 20 industry partners, over 30 governmental agencies, and over 140 community and health advocacy organizations.

We are currently conducting research in primary care for older adults with funding from the CFN.

Age well logo

AGE-WELL encompasses many sectors, including industry, non-profit organizations, government, care providers, end-users, and academic partners from across Canada. This multi-sector organization is using high-quality research to drive innovation and create technologies and services that benefit older adults. The aim of AGE-WELL is to help older Canadians maintain their independence, health and quality of life through accessible technologies that increase their safety and security, support their independent living, and enhance their social participation.
