Better care, same team: Inside a pharmacist-led model changing cancer care

Manulife presents CareNext Collective, a monthly virtual forum hosted by the CareNext Coalition.

This month, the event explores what happens when pharmacists move upstream into clinical decision‑making, take ownership of patient panels and redesign follow‑up care to address growing capacity pressures in oncology.

Anthony Amadio, Director of Pharmacy, Jackie Diebold, Clinical Pharmacist, Oncology, and Dr. Mary DeCarolis, General Practitioner in Oncology will unpack how their pharmacist‑led, interprofessional model was designed, operationalized, and refined — and what it reveals about practicing to scope, building trust across teams, and rethinking how follow‑up care is delivered.

They will be joined by pharmacist Joey Champigny (Ambulatory Care), to explore how lessons from this model could inform care redesign in other WRHN programs and across the broader health system, including opportunities to rethink roles, expand team capacity, and improve care delivery across different clinical settings.
