Library Accessibility Services request

All students who submit a form will be offered a meeting / orientation date with the Library Accessibility Services (LAS) Coordinator.

Disability disclosure and/or submission of any supporting medical documentation is not required for LAS / Adaptive Technology Centre (ATC) use eligibility.

This form does not replace AccessAbility Services’ student LAS / ATC referral forms.
