• To book an eye exam at the Kitchener Clinic, call 519-888-4455 or book your appointment online. If you are booking for a child six and under, please call – these appointments cannot be booked online. Individuals with special needs are best served at our Waterloo location –  see above.

We are unable to book appointments by email. Please have your health card number ready when you call.
