Use this form if you are a University of Waterloo postdoc who is planning to take a pregnancy or parental leave (including adoption and surrogacy). Submitting this form will initiate contact with the Human Resources department, who will follow up via email with you for the next steps.

Is your spouse or partner's salary paid through tri-agency funding (i.e. NSERC, SSHRC, or CIHR funds)?
