Postdoc pregnancy and/or parental leave intake form

Use this form if you are a University of Waterloo postdoc who is planning to take a pregnancy or parental leave (including adoption and surrogacy). Submitting this form will initiate contact with the Human Resources department, who will follow up via email with you for the next steps.

Some sources of funding (e.g. NSERC) for postdoc parental leaves require the funding to be shared between partners if they are both receiving the same funding. Please share the following to support determination of funding eligibility and sharing requirements: 
Is your spouse or partner also planning to take parental leave?
Is your spouse or partner employed at the University of Waterloo?
Is your spouse or partner receiving funding through the same source as you (e.g. also receiving tri-agency fellowship)?
