How do I make a FIPPA request?
- Print and fill out the access request form (pdf)
- A fee of $5.00 payable by check or cash is required at the time of application. Please make checks payable to: University of Waterloo
To submit your request:
- You may submit a completed FIPPA request with the application fee using the following methods:

Please note there is a drop box located next to the front door of Legal & Immigration Services’ office, located on the third floor of Needles Hall (3007) for the deposit of Access or Correction Requests.
