Forms

The following forms are for current graduate students. 

Graduate Studies course drop/add

Use this form to:

  • Audit a course or to take a course as extra to your degree.
  • Enrol in a course that requires an override.

Use this form to:

  • Use a course not listed in the graduate studies academic calendar associated with your program to meet a course requirement of your program.

Change of enrolment status

Use this form to:

  • Request a change to your enrolment status (e.g. full-time to part-time, part-time to full-time, inactive, full-time off campus, co-op, or exchange)
  • Voluntarily withdraw from your program
  • Deadlines: Fall – October 15; Winter – January 15; Spring – June 15

Change of supervisor

Use this form to:

  • Request and receive approval to change your program supervisor.
  • Request and receive approval to add or remove a co-supervisor.
  • Clarify start and/or end of financial support.

Graduate program/plan change

Use this form to:

  • Change your current program and to be considered for advanced standing, including transfer credit for some or all courses taken and milestones or research completion in your current Waterloo program.
  • Change your supervisor, if applicable, because of the program/plan change.
  • Deadlines: Fall – October 15; Winter – January 15; Spring – June 15

Request for Program Extension

Use this form to:

  • Request to continue studies beyond graduate program time limits.
  • The first extension(s) when a student is within three terms beyond program term limits is approved at the Department/School level. Further extensions beyond three terms past time limits must be approved at the faculty level, by the associate dean, Graduate Studies.
