The following is a list of the most commonly used forms along with a brief description of when they should be used. Visit the Registrar's website for a complete list of forms.
Elective approval form: use this form if you want to take an elective that does not appear on the SCE, LE, or ATE list.
SE 499 approval form: use this form to take SE 499 as an ATE.
Course override form: use this form to take a course during co-op, an extra course during the school term, if you do not have the proper pre-requisites, if the course is full, or if the course conflicts with another course.
Course Selection Override form: use this form if you want to take a course during co-op, to take an extra course during the school term, or if you do not have the proper pre-requisites.
Plan modification/application for internal transfer form: use this form to add an option to your degree, or to transfer from one program to another.
Letter of permission form: use this form to take a course at another university.
Undergraduate notice of withdrawal: use this form to withdraw from the term or from the University.
Application to graduate - undergraduate studies: use this form as your application to graduate and submit to the registrar's office before the deadline
