How do I create a ticket?

I have a visitor scholar or a visitor student; and need him/her to have an account.

Fill out the following form by login to this page: https://uwaterloo.atlassian.net/servicedesk/customer/portal/2/group/3/create/317. You will need to know the person name, email address, if that person have an account or not, if he/she does what's her username, and the expiry date for this account.

I need to send a ticket directly to IST. What do I do?
