You must complete and submit this form at least 48 hours in advance to reserve the GMC Sierra, Chevy Silverado, or Toyota RAV4, to be used for transportation to a competition, workshop, tradeshow, sponsor visit or to pick up supplies for your team. These vehicles are not to be used for personal use (e.g., shopping for personal items) or 'team building' events (e.g., bowling), and must be returned to the University parking lot at night unless you are out of town.
Please note a team vehicle questionnaire must be completed prior to submitting this form to determine eligibility to drive the RAV4 and only those who have successfully completed the student team truck training are eligible to drive the GMC Sierra and Chevy Silverado.
A copy of a valid Ontario "G" driver's license for each driver must be on file with the SSDC Administrative Assistant before driving a vehicle. Your vehicle request cannot be processed without the license submission.
