How to make a counselling appointment
Returning patients
If you have had a low vision assessment at our clinic, call 519-888-4708 to make an appointment.
There is no extra cost for up to six counselling appointments per year after you have paid the fee for your low vision assessment. Additional sessions may be added based on need and funding. Fees may apply for longer-term counselling or form completion.
New patients
Please see our intake process. If you complete a written intake form, please indicate that you are interested in counselling and/or community resources. If you complete the intake process by phone, please indicate this verbally. We will contact you to schedule an appointment.
