Low Vision Service fees

The services we provide are unfortunately not covered by the Ontario Health Insurance Plan (OHIP).  However, coverage may be available through the Ontario Disability Support Program (ODSP), the Assistance for Children with Severe Disabilities Program (ACSD), Veteran’s Affairs or private insurance plans. Please speak with us for more information.

Fees as of 2025

The following fees have been in effect since July 1, 2023, and are subject to change.

Full Low Vision Assessment (see below for details) $240
Follow-up within year of full assessment $100
Ministry of Transportation testing $240
Manual visual field test $105
Automated visual field test $60
Assessment for head-mounted devices (following a full Low Vision Assessment) $150
Form reports $25 + HST
Basic letter $40 + HST
Customized report/letter $100 + HST

Included in the Low Vision Assessment Fee

The Low Vision Assessment Fee is an umbrella fee giving clients access to the following low vision rehabilitation services:

  • Assessment and prescription of low vision devices.
  • Support in applying for funding, e.g. through the Assistive Devices Program, ODSP, ACSD, and Disability Tax Credit.
  • Report of results and recommendations.
  • Information and referral for medical, social, educational, vocational and financial services.
  • Consultation with and referral to other specialized services within the Waterloo Eye Institute.
  • Community/government agency support resources.
  • Up to six counselling sessions, some of which may focus on funding applications or referrals to external resources.
