If you’ve been impacted by challenging situations or experiences or would like a consultation for departmental needs, please complete the intake form below. We can connect you with services and resources and/or direct your inquiry to the appropriate persons within our office.
We want to let the campus community know that between December 15 and January 30, 2026, there may be significant delays in our response to disclosures submitted through the Advocate system.
During this period, individuals may experience a wait of up to 10-15 business days before a member of our team is able to reach out to schedule an initial consultation. These delays are due to staffing shortages, holiday closures, and scheduled staff time off.
We recognize that waiting can be difficult, and we appreciate your patience and understanding during this time. If you need immediate or ongoing mental health support, we encourage you to connect with Campus Employee Health and Counselling Services or other available support resources.
Please know that every disclosure matters to us, and our team will be in touch as soon as we are able.

Complete our intake form if:
- You have experienced or witnessed identity-based harm, including, but not limited to ableism, antisemitism, homophobia, Islamophobia, misogynism, racism, sexism, and transphobia.
- You would like to invite us to provide education or training in your area or unit.
- You want to work with us on an awareness initiative or other event.
- You would like us to review your policies, programs, documents, and physical spaces with an equitable lens.
- You are seeking an equity review of your area or unit.
- You are seeking specific EDI-R resources.
- You have questions or would like information on our initiatives (e.g., all-gender washrooms, menstrual equity, Program Area Leads).
- You have other general inquiries.
Here is what you can expect from our intake process:
- Within 3 business days of completing the form, a specialist from our office will get in contact with you, unless you choose to remain anonymous. Please note that if you choose to submit an anonymous report, we will not be able to contact you.
- During your initial meeting or email exchange, the specialist will gather essential information from you to understand your situation and/or requests, including your desired outcome.
- Where appropriate, the specialist will also provide you with information on privacy, consent and confidentiality. For example, if your needs or requests are better served (fully/partially) by others, we will obtain your consent and permission to share information on your situation with them.
- If you are reporting an incident of identity-based harm or discrimination, the specialist will inform you of the informal and formal (see Policy 33) options available to you on campus, so you can determine the most suitable option and best course of action for you. The specialist will connect you with the appropriate decision maker.
- Once your needs and goals are identified, we will develop and implement actions, interventions or programs designed to address your specific request. Interventions may include providing educational resources; reviewing your programs, policies and processes; conducting warm referrals to other offices; or collaborating with community partners. These interventions are tailored to your unique circumstances and preferences, with the overarching goal of promoting positive outcomes and enhancing your overall well-being.
