Your answers will be completely confidential and combined with other students' responses. Your participation is completely voluntary and you can decline to answer any of the questions or withdraw from participating at any point. Upon completion of the survey, you will have the opportunity to enter a draw for the prizes listed above. In order to administer the prize draws, you will be asked to provide your email address. Your name and email address will be used for purposes of administering the incentive only, and will not be linked to the survey data in any way. Names and emails will be removed from the survey data file and stored separately then destroyed after incentives have been provided.
