Protocol deviation

Protocol deviations are unanticipated or unintentional changes to a study that have occurred after it has received ethics clearance and can be minor or major in nature.

Deviations are to be reported using the Deviation Form located on the animal users' Teams site. If you do not have access to this Teams site, please email oreaupp@uwaterloo.ca.

  • Major protocol deviations may impact the research protocol, AUP approval, and the integrity of research results. They usually cannot be anticipated ahead of time, and may involve additional follow-up actions to ensure the safety and welfare of the animals.

Animal incidents

Incidents that may have compromised the health or safety of an animal in an animal care facility or while conducting field work must be reported.

Incidents are to be reported using the Incident Reporting Form located on the animal users' Teams site.  If you do not have access to this Teams site, please email oreaupp@uwaterloo.ca.

When an incident has occurred that has compromised the health or safety of an animal, this must be reported to the ACC.
