Notice of Collection

Personal information submitted as part of the University’s Proof of Vaccination and Campus Check-in Program is collected under the authority of the University of Waterloo Act, 1972 (as amended), including sections 3, 4, 14(1)(e), 14(1)(g), and 14(1)(h), and because it is necessary to the proper administration of a lawfully authorized activity. Personal information is collected, used and disclosed in compliance with the Freedom of Information and Protection of Privacy Act (FIPPA), R.S.O. 1990, c. F. 31, as amended (s. 38(2), s.41, s42); and in accordance with University of Waterloo policies (e.g., Policy 46, Policy 34) and guidelines, as well as other applicable laws.

The personal information collected is necessary to ensure health, safety, and security on campus; to enable continuity of learning and work of the University community; to comply with public health recommendations, directives, orders, or legislation; and for institutional planning and statistical purposes. 

Access to the personal information is restricted to those University of Waterloo employees who require the personal information in the performance of their job duties and the proper discharge of the University’s functions. The personal information will not be shared with any third parties, except as required or allowed by law. The personal information will be retained and disposed of in accordance with FIPPA and the University’s records retention schedules, no more than 1 year after its last use for the above purposes.
