Banting Postdoctoral Fellowship: Waterloo supervisor endorsement form

Please use this form if you are a University of Waterloo faculty member endorsing an applicant for the Banting Postdoctoral Fellowship. If you are endorsing this applicant in conjunction with a co-supervisor, only you (as the primary supervisor) need to complete this form. Please do so in consultation with the co-supervisor.

 Completion of this form is mandatory even if you are currently working with the candidate and/or invited them to apply.

Before submitting this form, review the Banting Postdoctoral Fellowship letter of intent and CV you received from the prospective applicant and confirm their eligibility. If you have not received a letter of intent, please have the scholar submit the letter of intent form prior to completing this endorsement form.
