Program Change Request Form for Applicants

Use this form if you recently accepted your Offer of Admission to the University of Waterloo and want to be considered for a program change for the fall 2024 academic term. Note: Do not submit a Program Change Request Form for Applicants unless you are sure you want to be removed from your current program should you be granted a spot in your new program of choice.

Application deadline: June 20, 2024

Application requirements

• You must be a student pursuing undergraduate studies.
• You must have accepted your Offer of Admission through the Ontario Universities' Application Centre (OUAC) website by the stated deadline.
  • If you have been admitted to any program within the Faculty of Science and would like to change your Subject of Major Interest (SMI), DO NOT complete this form. Contact the Faculty of Science to discuss this change.
• You must meet the desired program requirements to be considered for a program change.
• Complete all required fields on the form. Refer to your Offer letter when necessary.
• For all other programs, Subject of Major Interest will be confirmed with you by your academic advisor during course registration. You will receive details at a later date.

Decisions on program change requests will only be made if there is space available in the program you are requesting.

Due to the competitive nature of some of our programs, some programs do not allow program changes. If the program you wish to be admitted into is not listed in the dropdown menu, it means it is not open for a program plan change request.
