This Survey Review form is for University of Waterloo stakeholders interested in conducting a non-academic research survey involving any UWaterloo sub-population (students, staff, and faculty). Survey reviews are usually fulfilled within two weeks of the request date. However, please note that the nature, complexity of the survey, and other commitments may impact request completion date. Every effort will be made to process your request in a timely manner. If you have additional questions or wish to learn more about surveys, please contact IAP.
Note: Surveys whose purpose is for academic research need additional ethics clearance by the Office of Research Ethics (ORE). Please consult the ORE decision tree or contact the ORE to determine whether your project requires a review by University of Waterloo Research Ethics Committee (REC) or through IAP. The University of Waterloo expects all surveys to follow standard practices in ethical behavior.
