Low Vision Intake Process

You don't need a referral to visit the Low Vision Clinic. Just give us a call!

You do not need a referral to visit the low vision clinic – just give us a call!  In addition to self-referrals, patients are referred to us from optometrists, ophthalmologists, school boards, Augmentative Communications Clinics, WSIB, etc.

Information needed

Before scheduling appointments, the Low Vision Clinic collects some additional background information and clinical documentation to ensure that the planned visit correctly anticipates the unique service demands of each individual patient.  Nupur can complete the forms over the phone with you or mail or email them to you.

Next steps

Our optometrists will review your intake information and develop a rehabilitation plan for you. The patient care coordinator will then contact you to schedule your appointment. You may be scheduled for more than one appointment in a day, or we may recommend multiple appointments over two or more days.

Please let us know about any scheduling accommodations you may need. Please notify us at least 24 hours in advance if you would like to cancel or reschedule your appointment.
