Low Vision Intake Process

You don't need a referral to visit the Low Vision Clinic. Just give us a call! 

Call 519-888-4708 to speak with our patient care coordinator or email LVclinic@uwaterloo.ca. They can help you complete an intake form (link to the intake forms page). By gathering information ahead of time with the intake form, we can ensure that your appointment is focused on meeting the goals that are important to you.

We will also need a report from your eye doctor. We want to ensure that you are still taking care of your eyes through a regular eye exam. The report also helps us make your visit here more efficient and effective. We can identify training and devices which might be the most beneficial for you before your appointment so we can focus on your goals and devices that would help. If you need help requesting a report from your eye doctor, please let us know.

NEXT STEPS

Our optometrists will review your intake information and develop a rehabilitation plan for you. The patient care coordinator will then contact you to schedule your appointment. You may be scheduled for more than one appointment in a day, or we may recommend multiple appointments over two or more days.

Please let us know about any scheduling accommodations you may need. Please notify us at least 24 hours in advance if you would like to cancel or reschedule your appointment.
