Submit this form to have an Alumni WatCard issued and mailed to you. IMPORTANT:

Do not use this form if you plan to use the PAC. You must go to the Watcard Office in person to get an Alumni WatCard with your photo on it.
Do not order an Alumni WatCard if you are an undergrad or a graduate student. You will need to retain your student card as it carries more privileges, and you can only have one WatCard at a time.
Do not use this form if you wish to pick up your WatCard in person. Pick up at the Alumni Office is not available. You can go to the Watcard Office in person to get an Alumni WatCard issued onsite.
