This form is for new students wishing to get a Physical WatCard, that have never had a card before. If you are a returning student who has had a card issued in the past, please visit our office in person.
Before applying, verify that your preferred name is correct in WatIAM. Failure to complete this step will result in a $20 reprint fee to update the name on your card.
