Disability Verification

The OSAP Disability Verification Form is for students attending any post-secondary school outside of Ontario and is used for BSWD and/or CSG-PDSE purposes. It is not a substitute for the University of Waterloo documentation requirements. 

AccessAbility Services requires verification of disability from a licensed health care practitioner i.e.:  psychiatrist, psychologist, psychological associate, or family physician who has in-depth knowledge of a student’s condition.  Our office will use the information in part, to determine appropriate accommodations, services and financial support. 

Permanent disability is defined as a functional limitation due to a disorder that restricts a person’s ability to perform daily activities necessary to participate in post-secondary studies and is expected to remain with the person for the person’s expected life.

Functional Limitation is any restriction to a person's ability to perform daily activities necessary to participate in post-secondary studies and is expected to remain with the person for the person's expected life or a determined duration.

Students may be asked for updated information where a disability is identified as temporary or where academic accommodations need to be re-visited over time to ensure that the student's needs are appropriately met.

Students may receive interim accommodations pending receipt of documentation.

For those eligible for OSAP/BSWD/CSG-PDSE

Upon receipt of your University of Waterloo verification of disability form, AccessAbility Services will notify Student Affairs and Financial Aid (SAFA) of your disability status (permanent, temporary).

Documentation can only be accepted via fax, mail, uploaded with your application or in-person. 

When uploading documentation please note:

  • we require a clear scan for readability so photographs of documentation cannot be accepted

Please print the appropriate form and take it to your practitioner for completion.
