Disability Verification

OSAP Disability Verification Forms will be accepted by AccessAbility Services as proof of disability. However, to support the development of an academic accommodation plan, an in-depth understanding of the student’ functional limitations is required. All students are therefore directed to have a registered health professional, qualified to diagnose a disability, complete the University of Waterloo Disability Verification Form applicable to their unique condition.   

Permanent disability is defined as a functional limitation due to a disorder that restricts a person’s ability to perform daily activities necessary to participate in post-secondary studies and is expected to remain with the person for the person’s expected life.

Functional Limitation is any restriction to a person's ability to perform daily activities necessary to participate in post-secondary studies and is expected to remain with the person for the person's expected life or a determined duration.

Students may be asked for updated information where a disability is identified as temporary or where academic accommodations need to be re-visited over time to ensure that the student's needs are appropriately met.

Students may receive interim accommodations pending receipt of documentation.

Documentation Not Accepted

  • Individualized Education Plan (IEP)
  • Photographs of documentation, screen shots

Documentation can only be accepted via fax, mail, uploaded with your application or in-person. 

When uploading documentation please note:

  • we require a clear scan for readability so photographs of documentation cannot be accepted

Please print the appropriate form and take it to your practitioner for completion.
