Application for panel representative positions

The President’s Advisory Committee on Student Mental Health (PAC-SMH) is seeking members for five topic panels: Student Experience, Student Services, Community Partners, Academic, and Mental Health Experts.   

Information about the panels

The Supporting Panel Guidelines informs of the goals, structure, process, and expected outcomes. Please review it prior to filling out this form. More information about the topics each panel is responsible for exploring is included in the Guidelines. If you have any questions that you would like answered before applying, please contact the Director, Campus Wellness and Chair of the Advisory Committee – Walter Mittelstaedt.

How the representatives will be selected

Some prospective members will be contacted based on past expression of interest in the PAC-SMH, while others will be selected based on information provided on the below webform. The information that you provide is a key factor in the final selection of the representatives on the various panels. Please provide thorough answers in the comments sections. The PAC-SMH will gain a better understanding of your suitability for the committee work if the information provided is robust - including relevant examples of your skills and experience as they relate to this position and help convey your motivation for applying.

Each panel will have a designated lead person to be determined before the final selection of members.

Notes

By submitting this application, it is assumed that you:

  • Will be able to meet the estimated time commitments for the panels, including 2 hours for a bi-weekly meeting from July to December 2017.
  • Understand the biweekly commitments may include participation in public events such as town hall meetings and focus groups.
  • Have spoken with your supervisor to obtain permission for involvement, if necessary.
  • Can participate in the panel by reviewing materials between meetings.
  • Are available for a kick-off event planned for July 27, 2017,  9:00 a.m. to 4:30 p.m. (including lunch).
