Sharing a survey on behalf of the Canadian Dementia Learning and Resource Network (CDLRN).
“Understanding the Needs and Gaps in Support for People Living with Dementia”.
This study aims to identify the gaps in the community-based supports and services to meet the needs of people living with dementia and their care partners. We hope that the findings will help inform the development and improvement of programs and services.
Participation involves completing an online survey that will take approximately 15–20 minutes (link provided below). The survey includes multiple-choice and short-answer questions. Your participation is completely voluntary, and you may skip any questions or stop participation at any time.
At the end of the survey, you will be asked if you are willing to participate in a follow-up interview. If you answer yes, you will be directed to another page where you can share your email address. If you choose to provide your email address, it will not be connected with your survey responses. Participation in the follow-up interview is also completely voluntary.
The survey can be completed independently through the online link below or by phone call, or virtually (e.g., by Zoom) with a research assistant. We will then ask those who complete the survey if they would like to participate in an interview to share more about their experiences.

This study has been reviewed and received ethics clearance through a University of Waterloo Research Ethics Board.
