This form needs to be completed and submitted with the research ethics application when:
- there is a real, perceived, or potential conflict of interest by any member of the research team in conducting the research as per University of Waterloo Policy 69
- the research is funded by industry and a clinical trial involving a drug, medical device or natural health product.
