AccessAbility Services (AAS) collects, stores, and uses personal and health information as is reasonably required to provide disability-related accommodation services under the authority of Ontario's Freedom of Information and Protection of Privacy Act (FIPPA) R.S.O. 1990, c. F. 31 as amended, the University of Waterloo's Policy 46 - Information Management, and other legislative requirements.
- AAS collects information from students about their disability, functional limitations, and other related information necessary to support accommodation planning for the student.
- AAS collects information about the student’s academic studies (faculty, program, level of study, enrolled courses) at the University of Waterloo and personally identifying information such as your name, student ID number, birth date, and contact information.
- AAS collects this information to provide accommodation services and supports to students with known or suspected disabilities and disabling conditions. Examples of services and supports include:
- Determining a student’s eligibility for accommodations
- Developing an individualized accommodation plan
- Determining eligibility for provincial student aid bursaries to assist with disability-related educational costs
- Facilitating referrals to on and off-campus supports
- AAS collects information from a student’s application for academic accommodations, medical documentation, and self-report.
- AAS may collect a student’s medical documentation in a variety of ways including using AAS’ online system, Moveit, email (access@uwaterloo.ca), fax (519-746-2401), or in person at AccessAbility Services, Needles Hall North, 1st Floor, Room 1401.
