There are several forms that you could use to register with our office that will assist us in the design of your individualized accommodation plan. Each form provides guidance on who may complete it. Please select the form that you feel is most relevant to you.

Functional Limitation Verification Form - this form is best used when a disability diagnosis is not appropriate, available or relevant (e.g. trauma, grief). This form may also be used in situations where you are not working with a physician on your assessment and treatment plan.

Please note that it may take your practitioner a number of visits with you before they are able to adequately assess your functional limitations and complete documentation. Once you've submitted your documentation to our office, it can take up to 14 days (2 weeks) for your documentation to be reviewed. Please note that this review time may be longer during peak times. We encourage you to plan accordingly and begin working with your practitioner right away. Obtaining documentation is your responsibility, including any fee associated with having documentation completed. If cost is a barrier, please contact us by phone: 519-888-4567, ext. 45231 or by email: access@uwaterloo.ca to schedule an appointment to speak with an Accommodation Consultant to discuss possible funding sources or other alternatives.
If you don’t have documentation to support your disability, that’s okay. We can talk you through what you need. What is most important is applying with our office if you believe you need accommodations – we can help you with the rest.

Documentation can be uploaded directly to your application via the file upload link (new students) or through your existing AccessAbility Services account (registered students). This video shows you step-by-step instructions on how to upload your documentation through your AccessAbility Services account. Documentation can also be submitted in-person (Needles Hall North, Room 1401), via Sendit or email to access@uwaterloo.ca, or via fax to 519-746-2401.
The OSAP Disability Verification Form is used by the Student Awards and Financial Aid Office as proof of disability for the purposes of your Bursary for Students with Disabilities (BSWD) application. However, to support the development of an academic accommodation plan, an in-depth understanding of your functional limitations within the post-secondary environment is required. If you only have the OSAP Disability Verification Form, that’s okay. We can talk you through what you need. What is most important is applying with our office if you believe you need accommodations – we can help you with the rest.
Your Individualized Education Plan (IEP) can help us learn a bit more about you, and about the supports you used in the past. However, updated documentation that verifies the functional limitations that you experience in the post-secondary environment, will be needed to be eligible for ongoing accommodations, as well as for specialized bursaries and awards. If you only have an IEP, that’s okay. We can talk you through what you need. What is most important is applying with our office if you believe you need accommodations – we can help you with the rest.
