The three patient care rotation courses are PHARM 430 (primary care), PHARM 440 (institutional) and PHARM 450 (elective).
Each course consists of three major components: clinical practice, interprofessional evaluation and a Community of Practice Assignment.
- Students will contribute to healthcare planning and medication management therapy, patient care, disease states, therapeutic knowledge and clinic management, and counselling patients at, for example, family health teams, hospital outpatient pharmacies, community pharmacies, community health centres or any other outpatient clinic.
Institutional (PHARM 440)
- Students gain further experience in general clinical skills, healthcare planning and medication therapy management, patient care, disease states, and therapeutic knowledge in an institutional setting — e.g., hospital inpatient pharmacies, long-term care facilities, correctional facilities, group homes or other institutional settings.
Elective (PHARM 450)
- Miscellaneous rotations that are clinically focused on direct patient care
- Assigned to either a primary care or institutional setting or any other approved practice site — e.g., specialty clinics, home care, family health teams and community health centres.
Patient care rotation learning objectives
- Apply core knowledge, skills and professional judgment to provide pharmaceutical care
- Communicate with diverse audiences, using a variety of strategies
- Collaborate with a full range of healthcare teams to provide effective, quality healthcare and while fulfilling professional obligations
- Manage the medication therapy of patients with common and complex medication therapy problems
- Advocate on behalf of the patient and the profession to advance healthcare models
- Honour role as a self-regulated professional through individual patient care and fulfillment of professional obligations

All assessments must be completed electronically within CORE ELMS (previously RXpreceptor).
