- Ensure your committee meets the following criteria:

Two examiners must not be the student’s supervisor(s)
- At least one examiner must be a regular faculty member of the Department of Physics and Astronomy
- At least two examiners must hold regular faculty appointments at University of Waterloo
- Examiners must hold a PhD or equivalent degree

- You are responsible for finding a time that works with all of your committee members before filling out this form
- Your committee members must submit their questions and answers to the Graduate Coordinator two weeks prior to the exam date
