This form is intended for the student to complete in order to register for PHYS 437 A or PHYS 437 B.

Only fill this form once you have contacted a supervisor and agreed on a project.
General procedures and rules for 437A and B

Please note:

• course registration will not be processed until all required supervisors have emailed their approval to the Physics Undergraduate Office.
• students are not automatically enrolled into PHYS 437B after completion of PHYS 437A.
  A new form submission is required for continuation in the PHYS 437 research project course.
• enrolment will not be processed before the Open Enrolment Period for the term the project will be completed in

The latest date to make final arrangements for your project and notify the Physics Undergraduate Office is the end of the second week of lectures.
